Job summary

HIA Health seeks a Care Coordinator (MA/CNA) to support Primary and Palliative Care programs by coordinating care for medically complex, chronically ill patients. Qualifications

Responsibilities

• Provide ongoing care coordination for complex, chronically ill patients across care settings.
• Prepare charts, coordinate lab tests, medications, and care plans.

• Assist transitions of care and ensure follow-up.
• Document CCM activities accurately in EMR.
