Qualifications

• Master's degree in Counseling, Social Work or Nursing required.
• At least two years of related hospice or acute care experience.
• Eligible for or able to obtain one of the following certifications: Thanatology (ADEC), CHP-SW, ACHP-SW, CHPN, ACHPN.

Responsabilités

• Provide bereavement services, palliative care, and spiritual support to patients nearing end of life and their families.
• Collaborate with the Advanced Practice Nurse for Ethics and Bereavement and with hospital staff to model compassionate care and support staff education.
• Assist in data collection for research on end-of-life decision-making in collaboration with the health care team.
• Facilitate bereavement support groups for families who have experienced loss.
• Participate in staff development programs and patient/family education on Bereavement, end-of-life decision-making, patient rights, palliative care and spiritual issues.
