Job summary

Vitality Hospice & Palliative Care seeks a compassionate Bereavement Coordinator for a hybrid, part-time role in New Jersey, providing emotional support, grief counseling, and resources to families and caregivers as they navigate loss, with remote and in-person visits.

The role includes individual counseling, grief support groups, memorial planning, and coordination with the hospice team, plus maintaining records and ensuring regulatory compliance.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Theology, or related field; Master’s degree preferred.
• Experience providing grief counseling or bereavement support.
• Hospice or palliative care experience preferred.
• Strong communication and organizational skills.
• Ability to travel locally with a valid driver’s license.

Responsibilities

• Provide pre-bereavement and grief counseling to patients, families, and caregivers.
• Assess emotional and bereavement needs and develop support plans.
• Provide ongoing bereavement support for at least 13 months after a patient’s passing.
• Coordinate individual counseling, grief support groups, and other programs.
• Plan memorial services and remembrance events.
• Maintain accurate bereavement records and documentation.
• Collaborate with nurses, social workers, chaplains, volunteers, and hospice team.
• Assist training and coordinating bereavement volunteers.
• Provide grief education and support to hospice staff.
• Ensure compliance with regulations and policies.
• Participate in team meetings and occasional on-call coverage.
