North Country Healthcare in New Hampshire seeks a Bereavement Coordinator and Clinical-Volunteer Liaison to work with the interdisciplinary team, provide emotional support, grief resources, and coordinate volunteer services for hospice and bereavement programs.

The role reports to the Hospice Program Manager and collaborates with Social Workers, IDT, and community partners to connect patients and families with services.

Qualifikationen

• Bachelor’s degree in an applicable field is required.
• RN licensure in the state of practice is required.
• At least one year experience in hospice, palliative care, or bereavement support preferred.
• Relevant certifications in death, grief, or counseling are encouraged.

Aufgaben

• Provide bereavement services and education to families and communities.
• Coordinate volunteer recruitment, training, and supervision within hospice programs.
• Work with IDT and Social Worker to plan and implement care and resources.
• Maintain documentation and HIPAA-compliant records of bereavement activities.
• Develop individualized bereavement plans and monitor outcomes.
• Support care transitions and community outreach regarding hospice services.
