Job summary

Compassus is seeking a Bereavement Coordinator to lead a structured bereavement program for patients and families during hospice care and for up to one year after death. The role emphasizes compassionate counseling, community outreach, and collaboration with the interdisciplinary team to deliver high-quality bereavement services.

Qualifications

• High school diploma or GED required; bachelor\'s degree in Counseling, Social Work, Psychology, Theology, Divinity, or equivalent strongly preferred.
• Minimum of two (2) years experience in grief, loss and/or bereavement support; equivalent training/education may be considered.

Responsibilities

• Participates as a member of the IDT, including development and implementation of the plan of care.
• Works with the hospice Social Worker in pre-bereavement risk assessment and care plan development for patient and family.
• Promotes and supports assessment of bereavement risk factors at admission and throughout care.
• Develops and implements the bereavement plan of care after the death of the patient.
• Identifies needs of non-family caregivers and ensures bereavement support for them.
