Job summary

Vitality Hospice and Palliative Care in New Jersey seeks a compassionate Bereavement Coordinator. The role includes providing emotional support, grief counseling, and developing bereavement programs for families, with a mix of remote and in-person visits.

The position requires a Bachelor’s degree (Master’s preferred) in related field, experience in grief counseling, and ability to work independently within an interdisciplinary team; travel locally is required.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Theology, or a related field, or specialized certification in bereavement counseling. Master’s degree preferred.
• Previous experience providing grief counseling, bereavement support, or working with patients and families experiencing loss.
• Hospice or palliative care experience preferred.

Responsibilities

• Provide pre-bereavement and grief counseling to patients, families, and caregivers.
• Assess the emotional and bereavement needs of families and develop individualized support plans.
• Provide ongoing bereavement support and follow-up for a minimum of 13 months following a patient's passing.
• Coordinate and facilitate individual counseling, grief support groups, and other bereavement programs.
• Plan and organize memorial services and remembrance events.
• Identify individuals at risk for complicated grief and provide appropriate referrals to community resources and mental health professionals.
• Develop and maintain accurate bereavement records, tracking systems, and documentation.
• Collaborate with nurses, social workers, chaplains, volunteers, and other members of the interdisciplinary hospice team.
• Assist with training and coordinating bereavement volunteers.
• Provide grief education and emotional support to hospice staff when needed.
• Ensure compliance with hospice regulations, company policies, and professional standards.
• Participate in team meetings and occasional evening or weekend on-call coverage as required.
