The University of Tennessee Medical Center Home Health, an affiliate of The Pennant Group, is seeking a Home Health Medical Social Worker / Hospice Bereavement Coordinator to provide psychosocial support to patients and families and to coordinate bereavement services for loved ones.

Qualifications

• Master of Social Work degree from a CSWE-accredited program.
• Current social work licensure as required by state regulations.
• Minimum one year of social work experience in healthcare, home health, or hospice.
• Experience or education in grief counseling, bereavement support, or end-of-life care preferred.

Responsabilités

• Conduct psychosocial assessments of patients and identify factors impacting care.
• Develop and implement individualized social work plans with the care team.
• Provide counseling and supportive services to patients, families, and caregivers.
• Coordinate referrals to community resources and access to services.
• Collaborate with hospice team and participate in IDG meetings.
• Assist with discharge planning and documentation.
