Concord Hospital is seeking a compassionate APRN or PA-C to join its Palliative Care team in Concord, New Hampshire. The role provides consultations and symptom management in inpatient settings, partnering with patients, families, and interdisciplinary colleagues to improve quality of life.

The ideal candidate will have a master's degree, NH APRN/PA license, DEA registration, and at least one year of palliative care or related experience.

Qualifications

• Master's degree in Advanced Practice Nursing or Physician Assistant.
• Current New Hampshire APRN or PA license.
• Current New Hampshire RN license for APRN role.
• DEA registration.
• Minimum 1 year of experience in palliative care, hospice, geriatrics, oncology, critical care, family practice, or related specialty.

Responsibilities

• Provide expert palliative care consultations and symptom management in inpatient settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Facilitate goals-of-care conversations and advance care planning.
• Support patients and families through complex medical decision-making and transitions.
• Prescribe and manage medications for palliative and hospice needs.
• Collaborate with physicians, nurses, social workers, chaplains, and care teams.
