Job summary

North Country Healthcare is seeking a Hospice Nurse Practitioner (APRN) to deliver comprehensive palliative and hospice care across patient homes, facilities, and clinics in New Hampshire. You will collaborate with the Hospice Medical Director, attending physicians, and the interdisciplinary team to evaluate patients, manage symptoms, and educate families on goals of care and advance directives.

The role requires an APRN licensed in NH with national certification, plus active BLS/ACLS/PALS.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Licensed as an APRN in New Hampshire.
• National Nurse Practitioner Certification.
• Hospice and Palliative Care Certification preferred.
• Completion of BLS/ACLS/PALS or equivalent per role.
• DEA License required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide compassionate hospice and palliative care to patients with terminal illnesses.
• Perform comprehensive hospice/palliative care assessments and develop care plans.
• Prescribe medications and manage complex symptom control.
• Collaborate with physicians, nurses, social workers, and other team members.
• Complete CMS hospice recertification documentation and related records.
• Maintain timely and accurate documentation in the EMR.
