Job summary

North Country Healthcare in New Hampshire seeks a Hospice Advanced Practice Registered Nurse to provide comprehensive palliative and hospice care to patients with terminal illnesses. Working with the Medical Director and the Interdisciplinary Team, the APRN evaluates patients, manages symptoms, and develops individualized care plans.

You will educate patients and families, coordinate care across home health and facility settings, ensure CMS compliance, and lead advance care planning discussions.

Qualifications

• Master’s or Doctoral Degree in Nursing from an accredited program.
• National Nurse Practitioner Certification.
• Hospice and Palliative Care Certification (preferred).
• Licensed as an APRN in New Hampshire with a DEA license.

Responsibilities

• Provide compassionate hospice and palliative care to patients with terminal illnesses.
• Assist with complex symptom management and end-of-life care planning.
• Educate patients and families on disease progression, treatment goals, and advance directives.
• Coordinate with physicians, nurses, social workers, and other team members to ensure quality, compliant care.
