Job summary

Dartmouth Hitchcock Medical Center is seeking an APRN for Home-based Health at Home hospice and palliative care in Lebanon, NH. The APRN will conduct holistic assessments, perform face-to-face visits, and participate in the hospice interdisciplinary team.

Candidates must be board-certified APRN with active national certification (Adult Nurse Practitioner, AG-ACNP, or Family NP) and NH/VT licenses with DEA. Fellowship in palliative care or 2+ years hospice experience preferred.

Qualifications

• Board-certified APRN with active national certification in preferred path (Adult NP, AG-ACNP, or Family NP).
• Fellowship in palliative care or 2+ years hospice/palliative care experience preferred.
• Eligible for NH and VT licenses and DEA.

Responsibilities

• Evaluate and manage palliative care needs, including pain and symptom management.
• Perform holistic assessments and face-to-face visits as part of hospice care.
• Collaborate with the hospice interdisciplinary team and other providers.
• Educate patients and families about plan of care and goals of care.
• Coordinate with home-based care staff and facilities as needed.
• Engage with Dartmouth Health at Home programs and services.
