Job summary

Concord Hospital is seeking an Advanced Practitioner to provide acute and chronic inpatient care in Concord, New Hampshire. Weekend coverage (days and/or nights) with supervision by physician; involved in rounding, consultations, and patient education.

Eligible candidates have completed an Advanced Practice Nursing Program, hold a New Hampshire license, a valid DEA license, board certification (AANP or ANCC), and ACLS certification, with critical care/inpatient experience.

Qualifications

• Completion of an Advanced Practice Nursing Program.
• Current NH license to practice.
• Current DEA license.
• Board certification: AANP or ANCC.
• ACLS certification.
• Critical Care/Inpatient experience.

Responsibilities

• Obtains patient health history, performs exams, and documents encounters.
• Orders, interprets, and follows up on diagnostic tests.
• Diagnoses acute and chronic conditions and coordinates care with consultants.
• Prescribes and manages medications within practice standards.
• Delivers preventive care education tailored to hospitalized patients.
• Reconciles medications at admission and discharge to ensure safety.
