This role provides consultative palliative care services primarily in our inpatient settings, partnering with patients, families, and interdisciplinary team members to enhance quality of life, support complex decision-making, and manage serious illness symptoms.This is an excellent opportunity for an experienced provider who is passionate about whole-person care, values meaningful patient relationships, and thrives in a collaborative environment.## ## What You'll DoAs a key member of our Palliative Care team, you will:* Provide expert palliative care consultations and symptom management for patients with serious, chronic, and life-limiting illnesses.* Conduct comprehensive patient assessments, including health histories and physical examinations.* Develop, implement, and monitor individualized care plans focused on patient goals, comfort, and quality of life.* Facilitate goals-of-care conversations, advance care planning, and discussions regarding treatment preferences.* Support patients and families through complex medical decision-making and care transitions.* Prescribe and manage medications related to palliative and hospice care needs.* Collaborate closely with physicians, nurses, social workers, chaplains, care managers, hospice teams, and community partners.* Coordinate seamless transitions across care settings, including long-term care, hospice, home care, and hospital environments.* Provide education and support to patients, families, caregivers, and healthcare professionals.* Maintain accurate, timely clinical documentation and ensure compliance with regulatory and billing requirements.* Participate in quality improvement initiatives, program development, and interdisciplinary team activities.## Communication and Relationship-Based CareSuccessful candidates will be comfortable navigating sensitive conversations and providing support during some of life's most challenging moments, including:* Discussing goals of care and advance directives.* Facilitating family meetings.* Supporting transitions from curative to comfort-focused care.* Discussing hospice eligibility and enrollment.* Addressing end-of-life decision-making and care preferences.* Providing emotional support to patients, families, and healthcare team members.## What You'll Bring### Required Qualifications* Master's Degree in Advanced Practice Nursing or Physician Assistant* Current New Hampshire APRN or PA license.* Current New Hampshire Registered Nurse license, for APRN role.* DEA registration.* Minimum of one year of experience in palliative care, hospice, geriatrics, oncology, critical care, family practice, or a related specialty.### Preferred Qualifications* Prior experience working in hospice or palliative medicine.* Strong communication and relationship-building skills.* Experience working with interdisciplinary healthcare teams.* Certification in Hospice and Palliative Care (or willingness to obtain).## Why Join Concord Hospital?At Concord Hospital, you'll join a team dedicated to delivering compassionate, patient-centered care that honors individual values, preferences, and cultural beliefs.
