Job summary

Trustbridge Hospice, a member of Empath Health, seeks a full-time Palliative Care APRN to deliver comprehensive assessments, symptom management, and care coordination for patients with advanced illness.

You will partner with palliative care physicians and an interdisciplinary team to address physical, emotional, social, and spiritual needs, aiming to improve comfort and quality of life in a patient-centered approach.

Qualifications

• Master’s degree in Nursing required.
• Current Florida license as an Advanced Registered Nurse Practitioner (APRN) and DEA registration.
• ACHPN certification preferred or relevant hospice/palliative nursing certification.
• Minimum of two years nursing experience; at least two years as an APRN in palliative/hospice/gerontology/oncology.
• CPR certification from ARC or AHA.

Responsibilities

• Conduct comprehensive health assessments and obtain medical histories.
• Diagnose, treat, and monitor acute and chronic conditions with physician collaboration.
• Order and interpret diagnostic tests with physician involvement.
• Develop and evaluate care plans, incorporating pharmacologic and non-pharmacologic interventions under Florida APRN Act.
• Educate patients and families to promote self-care and positive health behaviors.
• Collaborate with the IDT to align care with patient/family goals.
