Job summary

Empath Health in Florida is seeking a Nurse Practitioner to provide comprehensive health assessments, diagnose and manage diseases in collaboration with physicians, and lead patient-centered care in a hospice/palliative setting.

You will order diagnostics, craft care plans, and educate patients and families. A Florida ARNP license and DEA, plus CPR certification, are required; ACHPN is preferred. NPI and CMS billing experience are a plus as part of a mission-driven team.

Qualifications

• Requires Master’s Degree in Nursing or related field.
• Florida licensed Advanced Registered Nurse Practitioner.
• Current DEA license.
• CPR certification from Red Cross or AHA.
• ACHPN preferred.
• NPI provider status and CMS billing ability if applicable.

Responsibilities

• Perform comprehensive health assessments and obtain medical histories.
• Diagnose, treat, and monitor diseases in collaboration with a physician.
• Order, perform, and interpret diagnostic studies with physician collaboration.
• Develop and evaluate plans of care, including pharmacologic and non-pharmacologic interventions.

Job description

What you'll Do

• Performs comprehensive health assessments by obtaining medical histories and performing physical examinations
• Diagnoses, treats, and monitors acute and chronic diseases in collaboration with a physician
• Orders, performs and interprets diagnostic studies in collaboration with a physician
• Identifies implements and evaluates appropriate plans of care, including pharmacologic and non-pharmacologic interventions within the guidelines of the Florida APRN Practice Act
• Promotes positive health behaviors and self-care skills through education and counseling
• Recognizes, reinforces, and participates as an IDT member for palliative care patients by assessing and modifying the plan of care in collaboration with the IDT as necessary to achieve physical, spiritual, social, and psychosocial goals centered on the patient/family goals

What You’ll Need

• Education: Master’s Degree in Nursing or a related field
• Licensure: Current Florida license as an Advanced Registered Nurse Practitioner
• Current DEA (Drug Enforcement Agency) license
• Certifications: Valid CPR certification from the American Red Cross or American Heart Association
• Advanced Certified Hospice and Palliative Nurse (ACHPN) preferred
• NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements)
• Minimum of two (2) years nursing experience required
• Minimum of two (2) year experience as an APRN with experience in palliative care, hospice care, gerontology, or oncology practice preferred
