Job summary

Northwell Health seeks an experienced Registered Nurse to perform age-specific care using the nursing process. You will assess, diagnose, plan, implement and evaluate patient care while coordinating with physicians and the health team on the unit.

Responsibilities include comprehensive physiologic and psychosocial assessments, formulating care plans, administering medications, documenting care, and contributing to discharge planning.

Qualifications

• Graduate from an accredited School of Nursing is required.
• Associate’s Degree in Nursing is required.
• Bachelor’s Degree in Nursing is preferred.

Responsibilities

• Assesses the patient’s physiologic health status.
• Assesses patient’s psychosocial health status.
• Identifies patient outcome and develops individualized goals.
• Formulates plan of care and prioritizes actions.
• Implements the plan of care and documents care.
• Evaluates care effectiveness and outcomes.
• Collaborates with care team and delegates as appropriate.
• Performs Decubitus care (GI/skin care) and skin management.
• Administers medications correctly and safely.
• Participates in patient and family education.
• Performs related duties as required.
