The VA Greater Los Angeles Healthcare System is seeking an experienced Registered Nurse to serve in Primary and Ambulatory Care at the Ventura CBOC location. The RN functions as a member of the Patient Aligned Care Team (PACT) — promoting improved health outcomes through care management, coordination, and collaboration.

The RN provides Veteran-centered care consistent with VHA goals and reports to the Nurse Manager.

Formación

• English language proficiency required.
• Graduate of an accredited nursing program with RN licensure.
• Possession of a current, full, active, and unrestricted RN license in the United States.

Responsabilidades

• Coordinate care for ambulatory clinic patients within the PACT model.
• Assist the provider with procedures and diagnostics as needed.
• Lead and supervise nursing staff in the clinic setting.
• Document patient care clearly in the medical record and communicate treatment plans.
• Support quality improvement and patient safety initiatives.
• Ensure compliance with patient rights and regulatory standards.
