U.S. Department of Veterans Affairs seeks an Outpatient PACT Registered Nurse to organize team members and achieve desired patient outcomes through day-to-day case management in a Primary Care Clinic using the Patient Aligned Care Team model.

Qualifications include ACEN/CCNE-accredited nursing education, an active RN license in the United States, and licensure compliance. The position can be eligible for Education Debt Reduction Program (EDRP) repayment support.

Competenze

• Graduate of nursing program accredited by ACEN or CCNE.
• Current, full, active RN license in a U.S. state or territory.
• State licensure required; temporary appointment possible with later conversion.
• Foreign nursing graduates must have degree equivalency validation.

Mansioni

• Provide comprehensive nursing care and day-to-day case management.
• Coordinate with the Primary Care Clinic's Patient Aligned Care Team.
• Triage patient needs and deliver education to promote wellness.
• Document care, monitor outcomes, and adjust plans as needed.
• Refer patients to community resources and coordinate care across settings.
