Job summary

The Veterans Health Administration is seeking a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) to deliver competent, evidence-based care to primary care patients. Requirements include a nursing degree from an approved program, active RN license, and English proficiency.

Qualifications

• English language proficiency required.
• Graduate of a school of professional nursing approved by accrediting bodies.
• Active RN license in US state (current, full, unrestricted).

Responsibilities

• Provide evidence-based care to primary care patients within the PACT model.
• Coordinate care and supervise licensed nursing staff as applicable.
• Lead health promotion, disease prevention, and chronic care management.
• Participate in professional development and quality improvement activities.
• Collaborate with interdisciplinary team to ensure safe, high-quality outpatient nursing care.
