Job summary

Veterans Health Administration, Department of Veterans Affairs is seeking a Registered Nurse to lead and coordinate care within the Patient Aligned Care Team (PACT). The RN will assess, plan, implement, and evaluate care, focusing on patient education, self-management, and satisfaction in a team-based outpatient setting.

The ideal candidate will have BSN, current RN licensure, and experience in ambulatory or primary care environments, with strong leadership and motivational interviewing skills

Qualifications

• English language proficiency is required by VA.
• Experience leading or coordinating care in an ambulatory or primary care setting preferred.
• Graduate of an accredited nursing program with active licensure in the United States.

Responsibilities

• Coordinate care for patients enrolled in the PACT across the care continuum.
• Lead an interprofessional team to deliver holistic, patient-centered care.
• Support patient self-management and wellness goals using motivational interviewing techniques.
• Facilitate communication between physicians, nurses, and other health professionals to ensure seamless care.
• Ensure continuity of care across inpatient and outpatient settings.
