The RN provides direct Veteran care and coordinates with licensed staff, delivering gender-specific, evidence-based care within the Primary Care Department. The role includes patient panel management, health promotion, education, and coaching on wellness and chronic disease management.

Responsibilities

• Provides direct Veteran care and care coordination within the Primary Care Department.
• Manages assigned panels, utilizes registries, and ensures appropriate evaluation and access to patient panels.
• Delivers health promotion, education, and coaching on wellness, disease prevention, and chronic care management.
• Leads in quality improvement and outcomes utilization; supports evidence-based practice across the team.
