Job summary

CSO in Leominster, MA is seeking a PACT Case Manager to provide intensive case management to adults 18+ within the Program for Assertive Community Treatment. You will work to help clients maintain independence in the community and engage those hard to engage in behavioral health services.

Responsibilities

  • Provide motivational counseling, prompting, and intensive support to consumers during psychiatric crises or transitions; assist with personal hygiene, grocery shopping, meal prep, and household activities.
  • Educate and support clients in identifying community resources, including mental health, addiction, and medical services.
  • Provide skills training in daily living activities, including basic and interactive skills, in vivo training, and placement development; include communication and conflict resolution.
  • Assist in developing a natural support network of family and friends.
  • Deliver crisis and intervention services as part of a mini-treatment team.
  • Assist clients in locating and maintaining housing, negotiating leases, and obtaining furniture; coordinate with landlords and housing authorities.
  • Provide recovery support services and help clients access treatment and community supports; explore treatment options and set goals.
  • Arrange transportation for consumers following program guidelines.
  • Maintain contacts with families, natural helpers, outpatient therapists, and others to support clients.

Job description

CSO is looking for a Case Manager to join our PACT team in Leominster.

Program for Assertive Community Treatment (PACT) is an evidence-based intensive outpatient treatment program designed to assist adults overcome the barriers to their recovery from severe and persistent behavioral health disorders.

This position will provide intensive case management to adults 18 years of age and older needing such support to maintain their independence in the community, or who have been hard to engage in the behavioral health system.

As a PACT Case Manager:

You Will:
  • Provide motivational counseling, prompting, and intensive support to consumers during psychiatric crises or periods of transition; monitor and assist clients with personal hygiene and grooming tasks, grocery shopping and meal preparation, and household activities
  • Provide education and support with identifying community resources including appropriate mental health, addiction and medical services, and accessing services of choice
  • Provide skills training and assistance in areas of daily living activities including basic living and interactive skills, providing "in vivo" skills training, side by side assistance, vocational and pre-vocational support to clients to include identifying interests and strengths and goals, and development of individualized placements. Assist in developing communication and conflict resolution
  • Provide assistance with developing and maintaining a "natural support network" of family and friends
  • Provide crisis and intervention treatment services as members of mini-treatment team
  • Assist, train and support clients in locating and maintaining decent and affordable housing, negotiating leases, and obtaining furniture and other necessary fixtures; facilitate cooperative relationships with landlords and housing authority as indicated
  • May provide recovery support services including exploring treatment recovery options with consumers, developing a set of goals and objectives in conjunction with the consumer based on identified needs, providing education and resources, and assisting consumers in accessing treatment and community supports
  • Provide transportation for consumers as appropriate, following program guidelines
  • Maintain appropriate contacts with family members, others in the "natural helping network", outpatient therapists, and others as necessary to support consumers
Qualifications
  • Bachelor's degree in a behavioral health discipline or related field
  • Must be able to work flexible schedule which may include evening and weekend work, as required by client needs
  • Valid driver's license and reliable transportation
