Job summary

U.S. Department of Veterans Affairs seeks an Advanced Medical Support Assistant to support a Patient Aligned Care Team (PACT) model or specialty care setting.

The selected candidate will work with a primary care team including providers, nurse care managers, and administrative staff to coordinate appointments and care coordination across inpatient and outpatient settings. Responsibilities include maintaining clinic schedules, processing referrals and follow‑ups, and ensuring accurate patient

Qualifications

• Evidence of experience or education must be documented on the resume with job title, duties, start/end dates and hours worked per week.
• Advanced knowledge of health care processes related to access to care and interdisciplinary care delivery.
• Ability to communicate effectively with a wide range of medical clinicians and patients.

Responsibilities

• Coordinate with the patient care team to review clinic utilization and ensure adequate clinic setup.
• Communicate with the patient, interdisciplinary team, VA medical centers, and external facilities.
• Enter information into the electronic record and manage follow-up care for consults and tests.
• Participate in team huddles, set priorities, and adjust work flow to meet patient needs.
• Educate providers about shared patients and ensure continuity of care.
