Job summary

San Ysidro Health is seeking a qualified social worker to provide direct case management for participants in the San Diego PACE program. You will conduct assessments, coordinate care, offer counseling, and connect participants with community resources.

You will participate in interdisciplinary team activities, maintain records, and advocate for participants, ensuring services comply with policies and laws.

Qualifications

• Master's Degree in Social Work required.
• Valid California driver’s license with access to insured vehicle.
• Counseling theory and techniques beneficial.
• Strong organization and project management abilities.
• Excellent interpersonal skills and teamwork.
• Ability to work independently and meet deadlines.
• Analytical thinking and problem-solving abilities.
• Practice within licensure boundaries.

Responsibilities

• Participates in initial assessments and care planning with the Interdisciplinary Team.
• Maintains current written case management records and reassessments.
• Provides in-home assessments and social service assessments.
• Coordinates 24-hour care delivery and advocates for participants.
• Delivers individual and family counseling; leads group counseling activities.
• Refers participants to community agencies and acts as liaison.
• Maintains knowledge of local services and geography.
• Represents the agency in external organizations as needed.
• Documents participant changes in medical records and participates in team meetings.
