Job summary

Family Health Centers of San Diego is looking for a social services assistant to help participants access community resources and coordinate care across multiple sites. You will support case management activities, documentation, participant follow-up, and service coordination to help individuals remain safely in the community.

Under supervision of Social Workers and PACE leadership, you will liaise with families, caregivers, and community agencies; assist with transitions of care, scheduling

Qualifications

• Bachelor’s degree preferred.
• Current CPR (BLS) or ACLS or ATLS required.
• Valid CA driver’s license and own car; travel between sites is essential.

Responsibilities

• Assist participants with applications to Medi-Cal, CalFresh, housing, transportation and other services.
• Work with Social Workers on case management and follow-up for participants and caregivers.
• Coordinate referrals and maintain compliant case records and documentation.
