Job summary

San Ysidro Health is seeking a Social Worker to provide direct case management and social services to participants in the San Diego PACE program. You will screen, assess, counsel and refer participants and families, and support inter-disciplinary team care planning.

Responsibilities include in-home assessments, coordinating 24-hour care delivery, advocating with community agencies, and maintaining up-to-date records. A Master’s in Social Work and a California driver’s license are required.

Qualifications

• Master's in Social Work from an accredited program.
• Excellent oral and written communication skills.
• Ability to work with diverse populations and coordinate care across disciplines.

Responsibilities

• Participates in initial assessments and care planning with the Interdisciplinary Team.
• Maintains accurate case management records and reassessments.
• Provides in-home social work assessments and counseling for participants and families.
• Coordinates 24-hour care delivery and connects participants with community resources.
• Advocates for participants and collaborates with external agencies to support services.
