San Ysidro Health seeks a Social Worker to support the Home and Community Based Alternatives Waiver Program. You will develop and monitor plans of treatment, perform in-home and telehealth assessments, and coordinate services to protect health and wellbeing of high‑risk participants.

You will partner with care teams to implement POT goals, arrange backup care when needed, and provide counseling and advocacy. Strong organizational and communication skills are essential.

Qualifications

• 2 years in a managed care health plan, utilization review, case management, or discharge planning.
• Experience in transitional care and acute care settings is valued.
• Knowledge of EHR systems; Epic preferred.

Responsabilités

• Develop and monitor the Plan of Treatment (POT) for HCBA Waiver participants.
• Conduct comprehensive health and psychosocial assessments with a focus on medical, functional, and social needs.
• Coordinate services, arrange training, and identify community resources to meet POT goals.
• Provide information, education, counseling, and advocacy to participants and families.
• Maintain accurate case management records and timely documentation.
