Job summary

Qualifications

• Bachelor's degree in Social Work, Gerontology, Psychology, or Marriage Family Therapy.
• Valid California driver’s license with access to an insured vehicle.
• 2 years of experience in managed care health plan, utilization review, case management, or discharge planning.
• 2 years of experience in transitional care and acute care settings.
• Strong communication and interpersonal skills; culturally sensitive.

Responsibilities

• Manage a caseload to develop and monitor the plan of treatment with a focus on health outcomes.
• Conduct comprehensive health and psychosocial assessments with RN collaboration.
• Develop POT goals and coordinate services and authorizations for waiver supports.
• Provide information, education, counseling, and advocacy to participants.
• Maintain accurate case management records and timely documentation.
