Job summary

AltaMed Health Services Corp. is seeking an Activities Coordinator to join the PACE IDT, designing and leading activities that promote physical, cognitive, social, and emotional well-being of participants.

The role emphasizes engagement through arts, movement, music, and community outings. Responsibilities include assessments, care planning, and ensuring adherence to program guidelines, with potential for remote activities based on participant needs.

Qualifications

• Minimum of a High School diploma or equivalent.
• Bachelor’s degree desirable.
• At least 1 year of experience in a social or recreational program for a frail or elderly population (preferred).
• Experience with dementia care is highly desirable.
• Completion of 36 hours of Activity Leader course approved by the State of California within 30 days of hire.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• Experience in providing virtual activities is preferred.
• A valid BLS certification or higher is required per AHA/Red Cross guidelines.

Responsibilities

• Design, implement, and oversee engaging activities tailored to PACE participants’ well-being.
• Actively participate in Assessments and Care Planning within the IDT.
• Incorporate arts & crafts, animals, sports, games, dance, music, and community outings.
• Adhere to PACE program guidelines and organizational policies.
• May use remote technology depending on participants’ comfort and interests.
