AltaMed Health Services Corp. is seeking an Activities Coordinator to join the PACE IDT and lead engaging programs for participants.

You will plan, implement, and oversee activities that support physical, cognitive, social, and emotional well‑being, using a range of methods from arts to community outings. The role requires coordination with the care team, adherence to PACE guidelines, and the ability to adapt activities to participant needs.

Qualificações

• Minimum of a High School diploma or equivalent.
• A bachelor’s degree is highly desirable.
• Minimum of 1 year of experience in a social or recreational program for frail or elderly population (preferred).
• Experience with dementia care is highly desirable.
• Completion of 36 hours of Activity Leader course approved by the State of California within 30 days of hire.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• Experience in providing virtual activities is preferred.
• Valid BLS certification or higher following AHA/Red Cross guidelines.

Responsabilidades

• Actively participate in care coordination activities, including assessments and care planning.
• Design, implement and oversee a variety of engaging activities tailored to physical, cognitive, social, and well‑being needs of PACE participants.
• Utilize arts and crafts, animals, sports, games, dance, music and community outings to promote a stimulating environment.
