Job summary

United Health Centers of the San Joaquin Valley in Fresno, CA is recruiting a PACE Registered Nurse (RN) to join our new PACE location. The RN will assess, coordinate, monitor, and provide clinical care to PACE members under primary care supervision.

The role requires CA RN licensure and ACLS certification, with a preference for candidates experienced with elderly populations. Bilingual English/Spanish is a plus, and strong computer and interpersonal skills are essential.

Qualifications

• Licensed as a Registered Nurse in CA.
• ACLS certification required.
• One-year experience as a Registered Nurse; experience with an elderly population preferred.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Provide care to PACE participants within scope of licensure and PACE model of care.
• Collaborate with Interdisciplinary Team (IDT) to ensure care plans are followed.
• Administer medications and manage medication systems.
• Perform phlebotomy procedures as needed and document nursing care.
• Mentor new staff hires and support on-site orientation.
• Schedule and coordinate med passes and appointments for clinicians.
