Job summary

Golden Valley Health Centers is seeking a PACE Adult-Gerontology Nurse Practitioner for the Central Valley Modesto PACE Center. You will perform assessments, create treatment plans, and coordinate care across clinic, day center, homes, and residential settings under physician supervision.

Requirements include RN license (California) and Family Nurse Practitioner degree. Bilingual English/Spanish preferred, with a focus on evidence-based practice and chronic disease management within GVHC’s PACE

Qualifications

• Bilingual English/Spanish is highly preferred.
• Experience in primary care and geriatrics is required.
• Strong communication and teamwork skills.

Responsibilities

• Obtain medical histories and perform physical examinations.
• Order labs and imaging as indicated for care planning.
• Diagnose and treat primary care conditions per protocol.
• Coordinate participant care with the Interdisciplinary Team (IDT).
• Maintain complete documentation in the EMR and update care plans.
