Job summary

North East Medical Services in San Francisco seeks a PACE Rehab Team Lead to guide the Rehabilitation team (PT, OT and other staff) in planning, implementing and overseeing rehabilitation services.

You will supervise daily operations, perform therapy roles as needed, coordinate care plans, train staff, and liaise with administration to ensure compliant, high-quality care for participants.

Qualifications

• Bachelor, Master or Doctoral degree approved by the California Board of Physical Therapy or Occupational Therapy.
• Licensed to practice as a physical therapist or an occupational therapist in California.
• Current BLS and First Aid required.

Responsibilities

• Oversees the quality of care and safety of participants at the Rehab Center.
• Provides daily supervision of operation and activities of Rehabilitation services and oversees rehab team.
• Performs roles and responsibilities of PT or OT.
• Provides direct therapy for participants.
• Conducts initial and periodic assessments to determine participants’ care needs, preferences, and goals.
• Onboards and coordinates training of new staff and provides ongoing training as needed.
