Job summary

Fallon Health seeks an experienced rehabilitation leader to oversee the PACE-related Rehab Department, ensuring high-quality services and efficient operations within Summit ElderCare. You will guide the rehab team, coordinate with the IDT and caregivers, and drive compliant, patient-centered care across settings.

The role requires MA licensure in Physical or Occupational Therapy, CPR certification, and a proven track record in program management, staff development, and regulatory reporting.

Qualifications

• Graduate of an accredited program in Physical or Occupational Therapy.
• Licensed as a physical or occupational therapist in Massachusetts.
• CPR certification or willingness to obtain.

Responsibilities

• Oversee the Rehabilitation Department for the PACE program across Fallon Health sites.
• Coordinate rehabilitation services and care plans with the IDT and caregivers.
• Supervise staff, including hiring, training and performance evaluations.
• Ensure timely documentation of care, claims processing and compliance with FH/SE policies.
• Lead utilization management targets and monitor cost of care for rehabilitation services.
• Collaborate with Home Care Manager on home safety evaluations and equipment needs.
