Job summary

Neighborhood Healthcare seeks a Rehab Manager to lead the PACE rehabilitation department, supervising physical therapy, occupational therapy, and speech-language pathology services. The role ensures regulatory compliance, quality care, and effective staff leadership across clinical disciplines.

Responsibilities include staffing, training, and coordinating department operations, with a focus on home health services, IDT processes, and patient-centered planning in a community-based setting.

Qualifications

• Graduation from an accredited Occupational Therapy or Physical Therapy program.
• Licensed Occupational Therapist or Physical Therapist in California.
• 5 years of experience as OT or PT.
• Graduation from a COTA/PTA program.
• California-licensed therapy aide.
• 5 years of experience as COTA/PTA.
• Current Basic Life Support (BLS) certification required; CPR/First Aid renewal annually.

Responsibilities

• Oversee rehabilitation department operations and supervision of therapy team.
• Coordinate and supervise OT/PT/SLP services across settings.
• Ensure compliance with CMS, PACE, and DHCS requirements.
• Manage staff onboarding, training, and performance management.
• Monitor staffing, workload balance, and service coverage.
• Oversee DME processes and in-home equipment safety.
• Maintain accurate patient records and care plans.
