Qualifications

  • Current BLS and First Aid required.

SUMMARY OF POSITION

Working under the supervision of the PACE Center Manager, the PACE Rehab Team Lead will lead the Rehabilitation team (PT, OT & other rehab staff) to plan, implement and oversee effective and efficient delivery of rehabilitation services.

ESSENTIAL JOB FUNCTIONS

Clinical Services

  • Oversees the quality of care and safety of participants at the Rehab Center.
  • Provides daily supervision of operation and activities of Rehabilitation services and oversees rehab team.
  • Performs roles and responsibilities of PT or OT.
  • Provides direct therapy for participants.
  • Conducts initial and periodic assessments to determine participants’ care needs, preferences, and goals. Establishes realistic and measurable goals and sets up treatment plans to administer rehabilitation treatment.
  • Participates in the development and revision of the participant’s plan of care as an active member of the interdisciplinary team.
  • Onboards and coordinates training of new staff to complete competency and other training materials, and provides on-going training as needed.
  • Serves as a liaison between rehab team and administration.
  • Promotes the growth of rehab staff in areas such as communicating with IDT, following regulations, decision making, and clinical skills.
  • Reviews and approves timecards, plans work schedules, assignments and coverages for vacations and holidays.
  • Implements Rehab team’s quality improvement strategies.
  • Remains alert to pertinent input from other team members, participants, and caregivers and updates the IDT promptly regarding any changes in participants’ condition or medical status.
  • Develops and implements a rehabilitation program to promote, restore or maintain participant’s optimum level of functioning, ensuring integration into interdisciplinary plan of care.
  • Provides training to professional and paraprofessional staff in body mechanics, use of equipment, bed mobility, transfers, gait patterns, home exercise program, etc.
  • Maintains necessary record keeping functions pertaining to participant care and progress.
  • Provides home visits & home health as needed.
  • Monitors customer service quality of Rehab team, conducts trainings and suggests improvement as needed.
  • Monitors to ensure that assessments and approved services for Rehab team are completed timely.
  • Utilizes Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
  • Performs other job duties as required by Manager/Supervisor.
General

Ensures that Rehab team:

  • Knows and adheres to the philosophy and goals of NEMS PACE.
  • Supports a quiet and safe environment for participants, visitors, and staff.
  • Maintains confidentiality of participant records, reports, and discussions.
  • Participates in formulation and maintenance of NEMS PACE policies and procedures, especially as they relate to rehabilitation program and services.
  • Participates in NEMS PACE Committee(s), as directed by the Center Manager.
  • Advises the Center Manager in ways and means to establish better accountability of NEMS PACE services to participants and referral sources.
  • Attends and participates in scheduled staff meetings and participant care meetings, as requested.
  • Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities.
  • Utilizes supplies and equipment economically.
  • Promptly informs the Center Manager of “unusual incidents.”
  • Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance when indicated; responds in a positive manner to constructive criticism; serves as a role model for students and staff.
  • Optionally holds membership in related professional organizations.
  • Complies with federal and state regulations.
  • Complies with code of ethics.
  • Complies with continuing education requirements.
  • Performs other job duties as required by manager/supervisor.
  • May provide after-hours on-call coverage to support PACE participants when the PACE Center is closed as needed for support.
QUALIFICATIONS

Required:

  • Bachelor, Master or Doctoral degree approved by the California Board of Physical Therapy or Occupational Therapy.
  • Licensed to practice as a physical therapist or an occupational therapist in California.
  • Must meet the educational requirements set by the California Board of Physical Therapy or Occupational Therapy.
  • Current BLS and First Aid required.

Preferred:

  • At least one year of clinical experience working with the older adult population.
  • Two to five years professional experience in Physical Therapy or Occupational Therapy in a health care setting (not including internship).
LANGUAGE
  • Must be able to fluently speak, read and write English.
  • Fluent in Chinese (Cantonese) preferred.
  • Fluency in other languages in an asset.
