Job summary

Valir PACE is seeking a Center Coordinator – LPN to support daily PACE center operations and clinical workflows. You will act as an on-site clinical resource, working with the RN Care Coordinator and IDT to coordinate services and ensure timely follow‑through on care plans.

You will assist with daily center activities, participant flow, charting, and communication between the center and clinical teams, while maintaining HIPAA compliance and a safe environment.

Qualifications

• Current LPN license in good standing.
• Current CPR certification.
• Minimum two years of clinical experience.
• Experience in geriatric care, primary care, or PACE preferred.

Responsibilities

• Coordinate daily center activities to support efficient participant flow and service delivery.
• Conduct routine rounds to identify participant safety, engagement, and environmental needs.
• Coordinate participant check-in and assist with maintaining center processes.
• Serve as an on-site clinical support resource for participant needs.
• Gather and document participant symptoms, vital signs, and changes in condition.
• Communicate changes in participant condition to RN Care Coordinators and providers for assessment and direction.
• Escalate urgent or emergent concerns per protocols.
• Assist with coordination of care transitions after hospital or SNF stays.
• Collaborate with Home Care Coordinators regarding scheduling and changes.
• Support documentation in the electronic medical record.
• Participate in IDT meetings and communicate updates, risks, and needs.

This role serves as an on-site clinical support resource and works collaboratively with the RN Care Coordinator, providers, and interdisciplinary team (IDT) to communicate participant needs, coordinate services, and facilitate timely follow-through on care plans.The Center Coordinator – LPN does not have direct supervisory responsibility and functions in a supportive and coordinating capacity.
