Job summary

PACE Partners of Northeast Florida seeks a Community Outreach and Participant Enrollment Coordinator to drive referral development and enroll 3–5 new PACE participants monthly. You will liaise with clinical teams and referral partners to ensure smooth enrollment and compliant outreach within CMS PACE guidelines.

Responsibilities include managing referral pipelines, conducting community education, and coordinating intake with Medicaid specialists as needed.

Qualifications

• Minimum three years’ experience in marketing, outreach, or social services activities.
• Minimum two years of documented sales success, preferably in selling services to medical professionals in the Jacksonville area.
• Clinical background preferred.
• Experience with frail or elderly populations preferred; training provided if needed.

Responsibilities

• Conduct compliant educational outreach about the PACE program model, eligibility criteria, benefits, and enrollment process.
• Facilitate timely referral intake and follow-up; collaborate with Marketing & Enrollment Manager and IDT to support enrollment growth.
• Maintain relationships with referral sources and manage a referral pipeline; educate partners on PACE services.
