Job summary

PACE Central Michigan seeks a dedicated professional to promote PACE to the community, generate referrals, and assist in enrolling participants. The role involves daily outreach to discharge planners, hospitals, aging services, churches, and other groups, coordinating intake visits, Medicaid applications, and enrollment tasks.

Qualifications

• Experience in senior care admissions and/or marketing is preferred.
• Excellent assessment and communication skills with seniors age 55 and older.
• Bachelor of Social Work or RN licensure.

Responsibilities

• Articulates PACE products and benefits to the market.
• Develops rapport with discharge planners, hospital staff, aging services organizations, churches, and others.
• Facilitates enrollment including intake visits, documentation, Medicaid processes, and in-home assessments.
• Prepares transition summaries and enrollment agreements for review.
• Maintains referral source contact lists and coordinates community presentations.
• Tracks referrals and enrollment data and reports to leadership.
