• Bachelor's degree or equivalent leadership/education in a related field.
• 3+ years in healthcare outreach, enrollment, sales, or related environment.
• 1+ year leading, coaching, or developing teams with proven impact.
• Experience with older adults, PACE, Medicare, Medicaid, or value-based care.
• Ability to analyze data to inform decisions and improve outcomes.

Responsibilities

• Leads and develops a high-performing Outreach and Enrollment team with accountability and growth.
• Develops strategies to drive participant growth and strengthen partnerships.
• Monitors performance and enrollment metrics using data to identify opportunities.
• Partner with center leadership to deliver a coordinated enrollment experience.
• Provide guidance on complex enrollment cases and compliance with regulatory requirements.
• Build strategic relationships with providers, community partners, and referral networks.
• Lead community outreach events to raise awareness of Habitat Health's PACE program.
• Ensure adherence to regulatory requirements and organizational policies.
