Job summary

Habitat Health is seeking an Outreach & Enrollment Supervisor in San Leandro to lead the Outreach and Enrollment team, drive participant growth, and ensure a seamless enrollment experience within the PACE program.

The role partners with center leadership, utilizes data-driven insights, and maintains compliance with federal and state requirements while expanding community partnerships and supporting organizational growth.

Qualifications

• Bachelor's degree in Business Administration, Healthcare Administration, Public Health, Marketing, or related field or equivalent leadership experience.
• Minimum of three (3) years in healthcare sales, outreach, enrollment, business development, care navigation, social services, or related healthcare environment.
• Minimum of one (1) year leading, coaching, or developing teams with demonstrated success.

Responsibilities

• Leads and develops a high-performing Outreach and Enrollment team with accountability and growth.
• Develops and executes outreach strategies to drive participant growth and strengthen referral partnerships.
• Monitors performance metrics and enrollment outcomes to identify opportunities and remove barriers.
• Partners with center leadership to deliver a coordinated enrollment experience and support transitions into PACE.
• Guides complex enrollment cases and ensures compliant operations.
