Job summary

Hospice Savannah is seeking a PACE Occupational Therapist to evaluate and implement holistic OT interventions for participants in the PACE program. You will assess needs, design interventions, and collaborate with the IDT to support meaningful daily activities and functional independence.

The role includes completing initial OT assessments, ongoing reassessments, and contributing to care planning within the PACE IDT framework.

Qualifications

• Requires Master’s degree in Occupational Therapy; Doctorate preferred.
• Minimum 1–2 years of OT work experience.
• CMS requirements: not convicted of offenses involving Medicaid/Medicare or health programs; not excluded from Medicare/Medicaid.

Responsibilities

• Evaluate participants and assess needs to plan OT interventions.
• Implement therapeutic interventions to improve functional independence.
• Complete OT assessments for new PACE participants and reassessments for existing participants.
• Participate in PACE IDT care planning and collaboration with the team.
