Job summary

PacificSource PACE LLC is seeking a qualified Occupational Therapist (OT) to join our interdisciplinary team. This role focuses on assessment, treatment, and interventions to improve or maintain older adults’ independence and overall quality of life across home and community settings.

As part of the IDT, you will collaborate with physicians, nurses, therapists, and social workers to implement individualized care plans and ensure regulatory compliance.

Qualifications

• Bachelor’s degree from an accredited occupational therapy program and OT licensure is required.
• CPR certification (BLS or ACLS) is required.
• A valid driver’s license is required.

Responsibilities

• Provide Occupational Therapy treatment and interventions to PACE participants across the continuum, including in the home and/or community setting.
• Conduct comprehensive assessments of participants to determine their functional abilities and needs.
• Actively participate in the interdisciplinary team by offering clinical expertise in team planning, problem-solving, and processes.
• Develop and implement personalized care plans aimed at improving or maintaining participants’ independence and quality of life.
• Guide participants in therapeutic, creative, and self-care activities to enhance their functional abilities.
• Review participants’ progress and adapt treatment plans as needed.
• Provide training, guidance, and supervision to OTAs or Personal Care Assistants, and participant/family members in carrying out the Functional Maintenance Program.
• Collaborate with other healthcare professionals to provide integrated care.
• Document in the electronic health record accurately, professionally, and in a timely manner.
• Visit participants in their homes or care settings as needed.

The PACE Occupational Therapist (OT) is a progressive, mission-driven role focused on improving the health and quality of life for older adults participating in the PACE Program This position provides assessment, treatment, and interventions, and oversees optimizing function and honoring what matters most to PACE participants. As a member of the PACE interdisciplinary team (IDT), the role actively participates in the development and implementation of the IDT Plan of Care across the continuum.

Essential Responsibilities

• Provide Occupational Therapy treatment and interventions to PACE participants across the continuum, including in the home and/or community setting.
• Conduct comprehensive assessments of participants to determine their functional abilities and needs.
• Actively participate in the interdisciplinary team by offering clinical expertise in team planning, problem-solving, and processes.
• Develop and implement personalized care plans aimed at improving or maintaining participants’ independence and quality of life.
• Guide participants in therapeutic, creative, and self-care activities to enhance their functional abilities.
• Review participants’ progress and adapt treatment plans as needed.
• Provide training, guidance, and supervision to Occupational Therapy Assistants (COTA) or Personal Care Assistants, and participant/family members in carrying out the Functional Maintenance Program.
• Collaborate closely with other healthcare professionals, including physicians, nurse practitioners, physician associates, nurses, physical therapists, social workers, and other members of the interdisciplinary care team, to provide integrated care.
• Meet all administrative requirements, including attendance at team and staff meetings, and recording and tracking OT rehabilitation equipment.
• Visit participants in their homes or care settings as needed.
• Serve on various internal and external committees as required or designated.
• Provide all services in a manner consistent with State and Federal regulatory requirements.
• Participate in team meetings, caregiver groups, and special events as needed.
• Document in the electronic health record accurately, professionally, and in a timely manner.
• Foster a safe and supportive environment conducive to the professional development of healthcare professionals.

SUCCESS PROFILE

Work Experience

A minimum of 3 years of experience in various healthcare settings is required, preferably in care management, geriatrics, chronic disease management, home health, or long-term care. Additionally, 1 year of experience working with frail, older adults is required.

Education, Certificates, Licenses

A Bachelor’s degree from an accredited occupational therapy program and OT licensure is required. CPR certification (BLS or ACLS) is required. A valid driver's license is required.

Knowledge

Employee(s) must have the ability to work effectively with individuals who have diverse health issues, concerns, and personalities. They should be proficient in accurately documenting case notes, assessments, contacts, and writing letters that may become part of legal records. Excellent organizational skills and experience with various computer software applications, including Word, Excel, PowerPoint, and Outlook, are essential. The capability to work independently with minimal supervision is required, along with exceptional communication skills, both verbal and written. Strong active listening skills are also necessary.

Staff Conditions

• Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
• Only act within the scope of his or her authority to practice.
• Have 1 year of experience working with a frail or elderly population must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
• Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.
• Be medically cleared for communicable diseases before engaging in direct participant contact.

Environment

Work inside in general office and clinical settings with ergonomically configured equipment. Travel is required approximately 20% of the time.

The maximum amount listed represents the highest possible salary for the role and should not be interpreted as a typical starting wage. Actual compensation will be determined based on factors such as qualifications, experience, education, and internal equity. Please note that the stated range is for informational purposes only and does not constitute a guarantee of any specific salary within that range.

Base Range: $74,966.03 - $123,693.95

Physical Requirements

Work is performed in a clinic setting with typical healthcare exposures, including contact with bodily fluids, infectious diseases, and cleaning chemicals; PPE is provided and required. The role involves stooping, bending, sitting, standing, and moving between clinical areas for extended periods. May assist participants with mobility, including guiding, supporting, escorting, and occasional transfer assistance using proper body mechanics. Must be able to lift, push, pull, or carry up to 35 pounds occasionally and perform frequent repetitive motions such as typing and operating medical or office equipment. Some positions require occasional travel to PACE sites, participant homes, or external appointments. Must be able to speak clearly, understand verbal instructions, and interact professionally with participants, caregivers, and staff.
