Job summary

BoldAge PACE is seeking an Occupational Therapist to join our interdisciplinary team in Holly Hill, FL. The OT will plan, organize, and implement therapy services for PACE participants, performing assessments, developing treatment plans, and coordinating care across center, clinic, and home settings.

You will educate participants and caregivers on safety and ADL strategies, supervise COTAs, and contribute to quality improvement and compliance, while maintaining licensure and ongoing professional

Qualifications

• Advanced degree in Occupational Therapy from an accredited school.
• Current license to practice Occupational Therapy in the state.
• Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting preferred.
• 1 year of experience with frail or elderly population preferred. If not present, training provided on hiring.

Responsibilities

• Plan, organize, and implement Occupational Therapy services for PACE participants as part of the interdisciplinary team.
• Conduct initial and periodic in-person assessments to evaluate functional abilities and needs.
• Develop, implement, and revise individualized treatment plans.
• Deliver OT interventions across settings including center, clinic, homes, and contracted facilities.
• Educate participants and caregivers on equipment use and home safety strategies.
• Maintain timely documentation of assessments, treatment, and progress.
• Supervise COTAs and provide mentorship to staff.

Education

Advanced degree in Occupational Therapy
