Job summary

Hospice Savannah Inc. seeks an LPN for the PACE program in Savannah, GA. The LPN will deliver general nursing care to participants, perform interventions under supervision, and triage medical conditions in clinic and by telephone.

Responsibilities include patient education, scheduling referrals, and coordinating with the RN and PCP for clinical decisions. Strong CMS/compliance knowledge and EHR proficiency are preferred.

Qualifications

• Current LPN license in Georgia in good standing.
• Experience with frail/elderly populations preferred.
• Competencies and tools required by CMS and PACE program.

Responsibilities

• Provide general nursing care to PACE participants in clinic and by phone.
• Perform nursing interventions under supervision of PACE providers.
• Escalate findings to RN or PCP for assessment.
• Schedule participant appointments for PACE clinic referrals.
