TALENT Software Services is seeking a Home Care Coordination support role in California to collaborate with an interdisciplinary team and supervise the coordination of home-based services for participants. The role involves safety evaluations, arranging meals, caregiver services, and DME while maintaining compassionate, person-centered care.

Responsibilities include completing enrollment care plans, maintaining EMR documentation, and regular communication with participants and families.

Competenze

• Must hold CNA and California HHA certificates with CPR/First Aid.
• Experience coordinating geriatric home health services and working in IDT settings.
• Strong computer skills including EMR, Excel, and Outlook.

Mansioni

• Coordinate IDT-approved meal delivery, DME, and caregiver services to participants.
• Complete enrollment and semi-annual care plans in the EMR.
• Conduct home safety evaluations and address changes of address.
• Document actions and service delivery status in logs and EMR in line with regulations.
• Communicate with participants, families, and healthcare providers to ensure coordinated care.
• Support nurses in coordinating meals, DME, and services as needed.
• Attend meetings and participate in committee activities as directed.
