The position requires a California CNA certification, at least 1 year of healthcare experience, a valid driver’s license, bilingual abilities (English plus Spanish/Mandarin/Cantonese preferred), and a current BLS

Qualifikationen

• Graduation from an accredited certified nursing assistant program with a current CNA certification issued by the State of California is required.
• Minimum of 1 year of experience practicing as a CNA or healthcare worker, and experience working with a frail or elderly population preferred.
• A driver's License is required.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Aufgaben

• The Home Care Coordinator supports and implements home care services for participants.
• Evaluates safe home environments through home visit assessments and coordinates with home caregiver vendors and family caregivers.
• Coordinates durable medical equipment and nursing home care; attends IDT and coordination meetings.
• Acts as liaison between the IDT and home care providers; demonstrates leadership and communication skills.
• Identifies and understands participants' medical and social needs to plan home care accordingly.
