Job summary

St. Paul’s PACE Corp in California seeks a Home Care Coordinator to perform in-home safety evaluations, coordinate assessments, and monitor care plan compliance under the Clinical Services Manager.

You will work with the Interdisciplinary Team to ensure environments are safe for care delivery, follow CMS guidelines, and document clearly across disciplines. This role emphasizes regulatory compliance, effective communication, and ongoing safety training.

Qualifications

• California CNA license is required.
• CPR and First Aid certification are required.
• Minimum of one year experience with frail or elderly population.
• Experience in home care, long-term care, or community-based services preferred.

Responsibilities

• Complete initial, semi-annual, and change-of-condition in-home safety assessments in compliance with CMS guidelines.
• Document all home safety assessments accurately within required timeframes.
• Attend plan of care meetings and complete in-home service plans for continuity of services.
• Monitor participant demographics and update addresses and contact details as needed.
• Schedule and complete home safety surveys before initiation of in-home services or within 30 days for others.
• Identify durable medical equipment needs and coordinate approval through Rehabilitation Department.
• Arrange referrals to light housekeeping and laundry vendors when indicated.
• Evaluate light housekeeping and laundry service appropriateness and communicate recommendations to supervisors.
• Provide detailed visit-specific information to home care staff (parking, pets, directions).
• Review the Welcome to PACE Home Care Services document with participants and obtain signatures.
• Attend in-service education, orientation, and skill training; complete first aid/CPR training within six months.
• Follow all company policies, OSHA guidelines, and confidentiality requirements.
• Conduct infection control practices and maintain a safe working environment.
• Participate in Quality Improvement initiatives and perform other duties as required.
