Job summary

Cross Country Search is seeking an Enrollment Specialist for an onsite role in Springfield, MA. Under the direction of the PACE Director, you will inform interested persons about the PACE Program and assist the potential participant and PACE staff in the enrollment process.

Key duties include following up with facilities for medical records, answering eligibility inquiries, meeting applicants at their home to explain enrollment steps, obtaining signed releases, and coordinating tours of the PACE

Qualifications

• Bachelor's Degree preferred.

Responsibilities

• Inform interested persons about the PACE Program and assist in enrollment.
• Fax, contact, and follow-up with medical facilities for medical records.
• Answer phone inquiries regarding PACE eligibility and enrollment.
• Meet with interested parties to explain all elements of the PACE program and enrollment process.
• Obtain signatures on release forms and communicate between participants, families and PACE staff.
• Schedule and provide tours of the PACE center and introduce staff to participants.
