Job summary

Golden Valley Health Centers seeks a PACE Adult-Gerontology Nurse Practitioner to provide high-quality care across clinic, day center, homes and residential settings. This position is based in Merced within the Central Valley PACE program.

The AGNP works under the supervision of the Medical Director/PCPs, performing assessments, planning treatment, coordinating care and delivering compassionate care to participants.

Qualifications

• Bilingual English/Spanish is highly preferred.
• Experience in clinical assessment and primary care.
• Ability to work within an Interdisciplinary Team (IDT).
• Strong patient communication and documentation skills.

Responsibilities

• Obtain medical history and perform physical examinations.
• Order labs, imaging and other studies as appropriate.
• Diagnose and treat primary care conditions under physician supervision.
• Document care in EMR and maintain up-to-date records.
• Coordinate participant care with physicians and care teams.
• Promote preventive care and age-appropriate screenings.
